What do I need to bring to my first visit?

  • Insurance card
  • Please arrive 15 min before your scheduled appointment time to allow time to complete paperwork and get you settled in
  • Your first visit will involve a cleaning and exam but more importantly allow us to get to know you as a person and tailor fit your treatment to your specific needs and concerns
